Project Summary

This is a 5-to-8 session educational program that helps diabetic individuals control their blood sugar levels in order to prevent complications from the disease. Participants learn about symptoms, complications, proper nutrition, including serving sizes and healthy cooking practices, physical activity, glucose monitoring, eye and foot care, as well as behavior modification to help them achieve lifestyle changes aiming at prevention of complications. In addition, it provides peer support to address some of the psychosocial needs. For the cost of $600 per year, the patient receives four to six physician visits, four sets of diabetic labs including HgbA1c, lipid panel, micro albumin, TSH, and a basic metabolic panel, one annual optometry and podiatry visit, two nutrition sessions with a Registered Dietitian, a glucose meter and sample strips (when available), and low cost medications.
