Opening February 2013!

North County Health Services is excited to announce that our family of health centers is growing! Since 1999, Carlsbad Family Medicine has been privileged to provide high-quality healthcare to the uninsured and underinsured for both adults and children, throughout Carlsbad. However, with the growing demand for NCHS’ health services, our tiny three exam room center quickly outgrew the ability to care for all of our patients, and a few years ago had to limit our patient care to pediatrics. The exciting news is that our new clinic will be more than doubling in size, so we can once again care for families, adults and children all in one location.

Our New Location – Your Health Home

The new Carlsbad Family Medicine location only about ½ a mile from the current location and is a different approach to the traditional health care model. This new approach utilizes the innovative model of Patient Centered Health Home (PCHH) and places the patients at the heart of the care we deliver, allowing the patient to experience:

• Minimal wait time
• Patient-based customer service
• Seeing your doctor  for each visit
• More time spent with your doctor
• Increased communication with staff and providers
• Follow up  from your doctor or staff member

 
In addition, our NCHS Patient Advisory Committee has helped design our new location to meet the needs of the each patient so we can deliver a care plan that benefits them and their family.

Seeing NCHS’ clear benefit to the community, the Carlsbad City Council unanimously approved a grant allowing us to move into our new location. Combined with the generous support of local “hero-donors” and fundraising proceeds, we will come close to getting all of the funds needed for the necessary equipment and furnishings.
